This survey is completely anonymous, and your honest input will contribute to making SNHD a better place for everyone. We appreciate your time and insights!

Get Your Code Ready
You’ve been provided a unique code to begin the survey. This code is completely anonymous and cannot be traced back to you by anyone in any way. Your confidentiality is important to us, so please feel comfortable sharing your honest thoughts. When you’re ready, please set aside enough time to complete the survey in one sitting, as the code can only be used once. You will need about 15 – 20 minutes to complete the questions. From the Director
In order to seek opportunities to improve our Facilities Department, we have engaged an outside independent contractor to undertake a complete survey of the Facilities Department. The confidential employee survey is a measurement tool designed to determine, among other things:
- how clearly we have set out our long-term direction and purposes
- how well we communicate, and
- how well we deal with our internal customers and employees.
In order to improve, we must commit to a constant process of internal review and analysis. One of the most productive ways to do this is to listen to your views on a broad range of issues. We believe that the time is right to undertake the important step of asking you, through this employee survey how well we are performing. I am encouraging you to take the survey and provide honest feedback for us to use in improving the Facilities Department.
